Central Auditory Processing Disorder Testing (CAPD Testing)
Central Auditory Processing Disorder (CAPD) is defined as an observed deficiency in one or more of these behaviors: sound localization and lateralization, auditory discrimination, auditory pattern recognition, and temporal aspects of audition. These include, temporal resolution, temporal masking, temporal integration, temporal ordering, auditory performance decrements with competing acoustic signals, and auditory performance decrements with degraded acoustic signals. (ASHA, 1996)
Behaviors of children considered "at risk" include:

*Frequently misunderstands oral instructions or questions
*Delays in responding to oral instructions or questions
*Says "Huh" or "What" frequently
*Frequently needs repetition of directions or information
*Frequently needs requests repetition
*Has problems understanding in background noise
*Is easily distracted by background noise
*May have problems with phonics or discriminating speech sounds
*May have poor expressive or receptive language
*May have spelling, reading, and other academic problems
*May have behavioral problems
When to test?
Consider testing if a child is doing poorly in school due to difficulties in learning through the auditory modality, poor listening skills, or difficulty understanding in the presence of background noise. Children who show behaviors listed above are considered at risk for central auditory processing disorders and are candidates for testing. In addition, children who show poor auditory performance on questionnaires like the CHAPPS are possible candidates for tests of central auditory function.
The following list was compiled in part with help from students taking a course in Auditory Processing Disorders. The reader is reminded that there are hundreds of websites on the subject of Auditory Processing Disorders, many of which provide good information, and many that are filled with misinformation. Parents and professionals are advised to be cautious when interpreting information from the internet. Remember that any person can purchase a web domain and publish information, whether they are knowledgeable or not, and none of the information on the internet is peer reviewed. Cochlear Implant Diagnostics
A cochlear implant is a small, complex electronic device that can help to provide a sense of sound to a person who is profoundly deaf or severely hard of hearing. The implant is surgically placed under the skin behind the ear. An implant has four basic parts:
1. A microphone, which picks up sound from the environment;
2. A speech processor, which selects and arranges sounds picked up by the microphone;
3. A transmitter and receiver/stimulator, which receive signals from the speech processor and convert them into electric impulses;
4. And electrodes, which collect the impulses from the stimulator and send them to the brain.
An implant does not restore or create normal hearing. Instead, under the appropriate conditions, it can give a deaf person a useful auditory understanding of the environment and help him or her to understand speech.

PACT: Program of Amplification for the Children of Texas
The Program for Amplification for Children of Texas (PACT) serves Texas children from birth through 20 years of age who have hearing loss that causes a problem in school.
PACT provides diagnostic and remedial services needed by children with hearing loss who are Medicaid eligible. Services are for children who have a hearing loss that is permanent.
Treatment for conductive hearing loss caused by middle ear problems is NOT covered by this program.
PACT has contracts with facilities throughout the state to provide these SERVICES:
* Evaluation by an audiologist
* Evaluation by an otologist
* Hearing aid evaluation
* Earmold(s) and hearing aid(s)
* Hearing aid counseling with child and family
* Follow-up checks of the hearing aid
* Hearing aid repair
* Earmold replacement
Hearing aids may be replaced when the current aids are no longer appropriate or are 5 years old.

Quick SIN
The QuickSIN is a speech-in-noise test that quickly and easily measures the ability to hear in noise. Speech understanding in noise cannot be reliably predicted from the pure tone audiogram or other standard audiometric tests. The QuickSIN test should be used on all adult patients as part of the audiometric test battery.

Real Ear Measurement
"One of the reasons many people are not happy with their new hearing aids is because their hearing aids have not been properly adjusted to fit their specific hearing losses.
Often, the audiologist or hearing aid dispenser adjusts your hearing aids according to the manufacturer's fitting algorithm (in the lab's software) for your hearing loss. Unfortunately, this algorithm does not necessarily reflect the hearing prescription you really need.
If your audiologist or hearing aid dispenser does not do "real-ear testing", he/she will never know whether or not your hearing aids are set up properly for your ears.
With real-ear testing, your audiologist puts a tiny microphone into your ear canal, then inserts your hearing aid. A computer runs some automatic tests and the results are shown on the screen.
The output shows both a graph of what your prescription should be, and what your hearing aids are really putting out. Now your audiologist can see what he/she needs to "tweak" in your hearing aids to properly give you the amplification you need at each of the speech frequencies.
How important is this? Consider this. A recent study using one hearing aid manufacturer's fitting algorithm was compared with the results of real ear testing. The shocking results show that less than 12% of the time were the results judged to be "clinically similar" when comparing the manufacturer's values to the actual real-ear measurements.
Nancy Aarts, Ph.D., the author of this study asserts:
'This study suggests that adult hearing aid fitting protocols should include the use of actual real-ear probe-microphone measures to verify hearing aid fittings. Reliance on [the manufacturer's protocols] may result in insufficient output of most hearing aid users, as well as over-amplification of high level inputs for some hearing aid users.'
What this means is that if you want to know that your hearing aids are fitted properly, you need to insist on having real-ear testing. If your audiologist doesn't do real-ear testing, find one that does. It's that important."

University of North Texas Training Center
Dr. Musani has been a participant in the University of North Texas' Audiology offsite training program for a number of years. Audiology students visit Dr. Musani's office on a regular basis to learn about Audiology in the "real world" setting.
Doctoral level students are able to experience live contact with patients who require audiological and hearing aid services under the direct supervision of Dr. Musani.
